Spouse / dependent update Your spouse and qualified dependent(s) are eligible to receive available benefits under your HRA VEBA plan. To avoid possible claims delays, make sure all required information is on file and accurate. Submit an Account Change Form containing your spouse and dependent information. Or, skip the form and do it online. Login to myHRA VEBA online and click Spouse/dependent information.
NOTE: Federal law requires the third-party administrator to have on file the full name, Social Security number, gender, and date of birth of all covered individuals. |
